No, a user account is not mandatory but recommended, as you will benefit from faster payouts (every 4 hours) and more. Read more here.

How can I get paid in Bitcoin?
NiceHash payouts are always in Bitcoin. If you want to use NiceHash without a user account (external user), you should use your Bitcoin wallet address for worker name. You will get paid to that wallet address.
